These are the simple  Steps for applying Name Change  in Gazette of India.

The following Documentsare required for Publication of name Change in Gazette of India.

1. An Application to the Controller Publication Department Civil Lines Delhi.

2. An undertaking duly signed by the applicant original Newspaper.

3. Prescribed Performa  duly typed with signature of applicant and two witnesses.

4. CD Certificate Containing the print Matter in MS Word and Type old Name in Place of Signature.

5. Two Self Attested Passport Size Photographs and Photo Copy of ID Proof (Self Attested)

6. A Certificate duly   signed  by the Applicant ie  declaring there  in that the contents of hard copy and soft copy are similar

7. An affidavit original copy attested by Notary

8. Receipt copy along with the Requisite Fees.

9. Must Require your Old Name ID

Means you need to provide any Government Old Name IdLikeAadhar Card/ Voter Card/Passport/ Pen Card with old name.
